Waiver
In In consideration for receiving permission to participate in the Firearms Class, I hereby release, waive, discharge and covenant not to sue Legally Armed In West Michigan, its officers, servants, agents and employees (hereinafter referred to as "releasees") from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the releasees, or otherwise, while participating in the Firearms Class, or upon the premises.
2. I am fully aware of risks and hazards connected with being on the premises and participating in the Firearms Class, , and I am fully aware that there may be risks and hazards unknown to me connected with being on the premises and participating in the GUN RANGE, and I hereby elect to voluntarily participate in the GUN RANGE, to enter upon the above named premises and engage in activities knowing that conditions may be hazardous, or may become hazardous or dangerous to me and my property. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of my being a participant in the GUN RANGE, whether caused by the negligence of releasees or otherwise.
3. I further hereby agree to indemnify and save and hold harmless the releasees and each of them, from any loss, liability, damage or costs they may incur due to my participation in the GUN RANGE, whether caused by the negligence of any or all of the releasees, or otherwise.
4. It is my express intent that this Release shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a Release, Waiver, Discharge and Covenant Not to Sue the above named. In signing this release, I acknowledge and represent that: A. I have read the foregoing release, understand it, and sign it voluntarily as my own free act and deed; B. No oral representation, statements or inducements, apart from the foregoing written agreement, have been made; C. I am at least eighteen (18) years of age and fully competent; or I am under the supervision of a fully competent adult; D. I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
RANGE RULES AND REGULATIONS Please read and initial the items below.
___1. No alcoholic beverages, drugs, or persons who have been consuming those items will be allowed on the range or premises.
___2. Shooter's glasses, tempered glasses or prescription glasses must be worn at all times while inside the firing area.
___3. Hearing protection must be worn at all times while inside firing area.
___4. Children must be supervised at all times while on premises. Children under the age of 10 will not be allowed in the firing area.
___5. No reloads, no armor piercing, tracer, or explosive ammunition or targets will be allowed.
___6. All firearms must be clear with the action open except while actively firing.
___7. All firearms must be clear and bagged or in a container before leaving the firing area.
___8. No person will be allowed forward of the firing line. Contact range staff if assistance is needed.
___9. Only persons who are actively shooting are allowed inside of the firing area.
___10. Only firearms in good working condition are allowed on the firing line. (user is held responsible)
___11. No more than two persons are permitted at each firing lane. Coaching may occur at the permission of range staff.
___12. All fired rounds must impact the target area. No impact into floor, roof, or side walls.
___13. All firearms are subject to inspection at any time by staff.
___14. Rapid fire must be approved by range staff.
___15. All firearms must be pointed down range in a safe manner at all times.
___16. No alcohol or tobacco use allowed on premises. I have read, I understand and I agree to abide by the above rules.
